By accepting this insurance you confirm that the facts below are true. We have relied on these facts, and on all information that you or anyone on your behalf provided, in agreeing to provide this insurance and in setting the terms and premium.

You must read this text to ensure that all the facts stated below are accurate and complete. If any of the facts stated below, or any of the information provided to us, is not correct or needs to be changed, you must tell us before the start of the period of insurance.

If there are any changes to this information during the period of insurance, you must tell us. When we are notified of a change, we will tell you if this affects your policy. If you do not inform us about a change, it may affect any claim you make or could result in your insurance being invalid.

Eligibility Criteria:

1. 1. You have no anticipated commercial product sales, revenue or fee income during the next 12 months other than consultancy revenue up to £150,000.
2. 2. You are domiciled solely in the United Kingdom of Great Britain and Ireland;
3. 3. Your business activities are within the following sectors;
   1. Pharmaceuticals
   2. Medical Device
   3. Biotechnology
   4. Nutraceuticals / dietary supplements
4. 4. You currently have no more than 20 employees;
5. 5. You do not handle group 3 or 4 biological agents as classified by EU Directives 2000/54/EC or 90/679/EEC
6. 6. You are not aware of any matter that is reasonably likely to give rise to any loss or claim, nor suffered any loss, nor has any claim been made against you in the last 3 years.

Risk Criteria:

1. 1. The business premises are constructed of brick, stone, concrete or other non-combustible material and are in a good state of repair;
2. 2. You have no more than 100 litres of Class I flammable/combustible liquids stored within any one laboratory or process area;
3. 3. The insured location has not flooded in the last 5 years and is at least 10 metres from any rivers, streams or tidal waters;
4. 4. The business premises are self-contained with a lockable entrance and in the case of shared premises a pass or verification is required to enter the building;
5. 5. You have never had an application for this type of insurance declined, cancelled or non-renewed
